There was once a Holy Grail War waged by Mages and Heroic Spirits in a town of Fuyuki. However, one Mage took advantage of the chaos of World War II to steal a Holy Grail. Several decades have passed, and the Yggdmillennia family, who took upon the Holy Grail as its symbol, defected from the Mages' Association. Furious, the Association sent forces to deal with the Yggdmillennia, but were defeated. With the Holy Grail system changed, war at an unprecedented scale breaks out.

The setting of Fate/Apocrypha is a parallel world to Fate/stay night where the Greater Grail was removed from Fuyuki after the Third Holy Grail War and disappeared for many years. Around the 2000s, around the same time as the Fifth Holy Grail War would have happened, the group that plundered the Grail, Yggdmillennia, openly declares their secession from the Mage's Association and that they are in possession of the Grail. The Association dispatches fifty magi to retrieve it, but all but one are instantly slaughtered by Lancer of Black. The one remaining manages to activate the reserve system of the Greater Grail, allowing for the summoning of fourteen Servants in total. 

This marks the start of the Great Holy Grail War in Trifas, featuring two different factions, the Black Faction, whose members are part of Yggdramillennia, and the Red Faction, whose members were sent by the Mage's Association with the exception of one from the Church. The Grail itself also summons Ruler to act as a mediator of the Holy Grail War.

Ten years after facing defeat in the Fourth Holy Grail War, Waver Velvet, now Lord El Melloi II, teaches classes at the Clock Tower—the center of education for mages. However, his new status as "Lord" comes with a caveat: obey the orders of Reines, the younger sister of the deceased Kayneth El Melloi, until she is old enough to rule the House of El Melloi. 
 
Waver, along with his mysterious apprentice Gray, takes on a series of cases assigned by Reines and the Mages Association. With each case proving to be more complex than the last, could there be more to the Clock Tower than meets the eye, and what secrets does Reines hide?

The series is set on the Far Side of the Moon Cell, where BB used her abilities to create the Sakura Labyrinth. She brought over various Masters and Servants from the Moon Cell Holy Grail War on the Near Side, some taken instantly before their deletion and others brought during the sixth week.

He is suddenly a full decade younger! Lord El-Melloi II has become Waver Velvet, his former self! Believing himself to be in the grip of a powerful, magically induced delusion, Lord El-Melloi II immediately begins investigating how this could have happened. On the day of the reunion, he makes his way to the gathering of his contemporaries, planning to confront whoever transformed his body and hoping to reveal their intentions. However, when Touko Aozaki arrives, things take a sudden turn...
