At the outset of the manga the team is in trouble, as there are not enough players to play a game. Additionally, the coach has left after a conflict with Fujiwara. However, Aikawa transfers to Mizuho and through his enthusiasm and sheer love of the game, infuses the few remaining members with a sense of purpose. 
Despite the fact that there are only five of them (i.e., they have no substitutes), they manage to get the coach of the girls' team to coach them as well. At first, she seems very strict, but later reveals her attachment to her "Dear Boys". Together, they do their best to make their dreams come true.

Dear Boys is a story about determination to excel in a sport in which you love and also have fun at the same time. The tension as underdogs in the whole prefecture with only 5 players. How this team struggle to win, setbacks, teamwork, a purpose in life. This story relates to our lives as we strive to do our best, not just in sports, school but also relationships between friends and ones we love.

The Mizuho team continues their dream to the National Tournament.  

In this special edition of the teenage basketball manga classic, "DEAR BOYS," Takumi Fujiwara and Ranmaru Miura of Mizuho Junior High meet Tsutomu Ishii and Kenji Dobashi from Takakura Junior High at a Mizuho High basketball game. The four boys aim to be the number one basketball player. Fans of "Dear Boys" will want to read about how the four first met, three years before the "DEAR BOYS" saga begins.
