Hisako is Mizuho's homeroom teacher to his class of 3-A and is the one of the only people in the entire school who knows that Mizuho is in fact a boy, other than Mariya, the headmistress and later Shion and Ichiko. She has a helping personality and even told Mizuho to come to her if anything was bothering him. She often gives out candy to her students.  Hisako makes an appearance again for the sequel Otome wa Boku ni Koishiteru 2: Futari no Elder, this time as Chihaya's homeroom teacher and the school's acting principal. She also knows his secret like Mizuho before him. Her appearance has changed since Mizuho's time at the academy, she has longer hair in particular.

Mizuho can't believe his ears when Grandpa's will is read out. Grandpa's final wish is for Mizuho to attend the same academy as Mizuho's mother. The catch is it's an all-girls school. With the help of friend Mariya however, he gets a makeover good enough to fool everyone at the academy. <br><br>

Otoboku 2 begins about two years after Mizuho's graduation. The main protagonist Chihaya Mikado was truant at his school because of harassment in the school and of a distrust in men. His mother cannot bear to watch him be in trouble, so she makes him transfer to Seio Girl's School, her old school. He dresses as a girl and introduces himself as Chihaya Kisakinomiya. Chihaya meets another protagonist Kaoruko Nanahara, who once rescued him from a playboy, and he begins to live in the dormitory building. He becomes the focus of public attention due to his attractive face and figure, and due to high grades at studies.<br><br>
